MVD NOTICE – NEW VEHICLE WRITTEN REMOVAL AUTHORIZATION FORM REQUIRED
The Arizona MVD has released an updated Vehicle Written Removal Authorization Form (Form 40-0206) that must be used effective May 29, 2026.
Please ensure that all drivers print and begin using the updated form immediately. The previous Vehicle Removal Authorization Form is no longer valid and will not be accepted.
Important: Per MVD requirements, cases submitted using the old form will be rejected.
The new form can be downloaded directly from the Arizona MVD website:
https://apps.azdot.gov/files/mvd/mvm/abandoned-vehicle-suitex/default.htm
Please distribute this information to all drivers and personnel involved in vehicle removals to ensure compliance and avoid rejected cases.
